Key Job Responsibilities

• Patient Intake and Referral Management: Develop and implement streamlined processes for patient intake, ensuring a smooth and welcoming experience. Manage referrals by liaising with healthcare providers and patients, ensuring accurate and timely coordination of care. Monitor and evaluate the efficiency of intake procedures regularly. Serve as the primary leader for patient intake, ensuring that all potential patients are successfully onboarded and that no referral opportunities are lost. Conduct follow-ups with patients to facilitate seamless transitions into care.

• Community Outreach and Relationship Building: Actively visit nursing homes, hospitals, and medical offices to establish and maintain strong relationships with referral sources. Promote the clinic’s services to healthcare providers, fostering trust and collaboration to ensure a steady stream of patient referrals. Serve as the primary point of contact for external providers, ensuring open communication and efficient coordination of patient care.

• Case Management and Care Coordination (“Navigator”): Facilitate comprehensive care coordination, including case management and patient navigation. Work closely with the clinical team to track patient progress, coordinate services across different care providers, and address barriers in delivery of care. Use a patient-centered approach to improve health outcomes.

• Administrative Support: Provide comprehensive administrative support including patient scheduling, record-keeping, and compliance with healthcare regulations.

• Updates and maintains medical records for in-center and home patients in neat, organized and professional manner to include periodic purging of outdated information

• Accurately ensures all first line of patient contact activities are properly conducted (consents, insurance information, documentation in EMR)

• Prepares hemodialysis charts for following day’s activity, post/file appropriate information

• Prepares lab slips and tubes for routine (monthly) and non-routine blood work; prepare, package and arrange for pickup lab samples to appropriate lab

• Maintains accurate patient data, including treatment, hospital, no-show, and absentee dates

• Coordinates completion of patient admission process

• Enters data into facility EMR system as appropriate

• Reviews and verifies transportation services for patient

• Leads new patients referrals (in and out)

• Visits local nursing homes, hospitals and medical offices to develop strong relationships with them and ensure patient referrals.

• Knows all emergency procedures

• Reports all results, incidents, complaints, or problems to Charge Nurse in a timely manner

• Performs duties of receptionist including sorting/distribution of mail, answering/routing phone calls, greeting/directing visitors, and other clerical duties

• Complies with HIPAA regulations

• Supports inventory management, updating records to ensure clinic is appropriately stocked

• Provide patient care as a CCHT when required, including assisting during shift changes, patient setup, treatment monitoring, and clinic closing activities.

• Supports LMSW, RD, FA, RN and other individuals that are part of the integrated care team

• Other duties as assigned

Qualifications

• Certified Clinical Hemodialysis Technician (CCHT) with active and valid certification preferred

• Associates degree in healthcare administration, Certified Medical Assistant, or High School diploma with relevant healthcare and/or dialysis experience.

• Versed well in phlebotomy, medical administration, etc. a plus.

• Strong organizational, leadership, and communication skills.

• Proficiency in healthcare management software and Google Suite and/or MS Office.

• Have full range of body motion and flexibility

• Able to do lifting typically under 20 lbs

• Able to climb, reach, kneel, stoop, and bend on a daily basis

• Able to carry on throughout the day with sufficient stamina

• Able to sit and/or stand for extended periods of time

Working Conditions

• In-person at Carmen [kidney HEALTH’s office located in San Antonio with visits to the Skilled Nursing Facilities located in the San Antonio, Austin and Houston regions when needed.

• Emphasis on creating an accessible, effective, and delightful healthcare experience.

• Occasional travel to other clinics, external storage facilities, doctor’s offices, hospitals, nursing homes, and other referring organizations.

Pay: $52,000.00 - $67,000.00 per year
