*Job Overview* We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. The MDS Coordinator plays a crucial role in ensuring the accurate assessment and documentation of resident care needs in compliance with federal and state regulations. This position requires strong organizational skills, attention to detail, and the ability to work collaboratively with various healthcare professionals. The MDS Coordinator will start from point of Admission process with the resident.

  • Conduct comprehensive assessments of residents using the Minimum Data Set (MDS) to determine their care needs.
  • Ensure timely completion and submission of MDS assessments in accordance with regulatory deadlines.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans based on assessment findings.
  • Monitor and review documentation for accuracy and compliance with HEDIS standards.
  • Utilize coding knowledge, including ICD-10, to ensure proper documentation of diagnoses and patient conditions.
  • Engage in utilization review processes to evaluate the appropriateness of care provided.
  • Maintain up-to-date knowledge of regulations related to nursing home operations and MDS requirements.
  • Provide training and support to staff regarding MDS processes and documentation standards.

*Experience*

  • Previous experience in a healthcare setting, particularly within nursing homes or hospitals, is preferred.
  • Familiarity with electronic health record systems such as PointClickCare.
  • Strong understanding of primary care principles and practices as they relate to patient assessments.
  • Excellent communication skills, both verbal and written, are essential for effective collaboration with healthcare teams.
  • Ability to work independently while managing multiple tasks efficiently.
