- Provide direct, individualized skilled nursing care to patients in home settings under the supervision of a Registered Nurse (RN).
- Administer medications, perform wound care, monitor vital signs, and execute physician-ordered treatment plans.
- Maintain timely, accurate clinical documentation (EHR) following each patient visit.
- Educate patients and their family caregivers on disease management, hygiene, and safe care practices.
- Collaborate closely with our multidisciplinary care team to report changes in patient conditions promptly.
*What We’re Looking For*
- *License:* Active, unencumbered LPN license in the state of practice.

- Current CPR/BLS certification.

Experience
- Minimum 1 year of clinical nursing experience (Home Health or Home Care experience preferred, but not required).

- Strong clinical assessment, wound care management, and clear communication abilities.
