  • Provide direct, individualized skilled nursing care to patients in home settings under the supervision of a Registered Nurse (RN).
  • Administer medications, perform wound care, monitor vital signs, and execute physician-ordered treatment plans.
  • Maintain timely, accurate clinical documentation (EHR) following each patient visit.
  • Educate patients and their family caregivers on disease management, hygiene, and safe care practices.
  • Collaborate closely with our multidisciplinary care team to report changes in patient conditions promptly.

*What We’re Looking For*

  • *License:* Active, unencumbered LPN license in the state of practice.

Certifications

  • Current CPR/BLS certification.
  • *

Experience

  • Minimum 1 year of clinical nursing experience (Home Health or Home Care experience preferred, but not required).

Skills

  • Strong clinical assessment, wound care management, and clear communication abilities.

Requirements

  • Valid driver’s license, reliable personal transportation, active auto insurance, and background check clearance.
