We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. The ideal candidate will coordinate patient care across various settings, ensuring seamless transitions from hospital to post-acute care and outpatient services. This role requires a strong foundation in acute and chronic care management, with an emphasis on patient-centered approaches, utilizing extensive clinical knowledge to optimize health outcomes. The RN Case Manager will collaborate with multidisciplinary teams, manage complex cases, and facilitate discharge planning to promote efficient and effective patient recovery.

Requirements
- Registered Nurse
- Valid RN license with current registration
- Experience in critical care settings preferred
- Proven experience in Level I Trauma Centers or Level II Trauma Center environments highly desirable
- Extensive background in ICU, emergency room, medical-surgical units, or post-acute care settings
- Strong knowledge of physiology, medical terminology, HIPAA compliance, and diagnostic evaluation procedures
- Expertise in discharge planning, utilization management, case management, and managed care processes
- Experience with patient assessment techniques including vital signs monitoring and clinical documentation
- Familiarity with hospital records systems and medical records management
