We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. The ideal candidate will coordinate patient care across various settings, ensuring seamless transitions from hospital to post-acute care and outpatient services. This role requires a strong foundation in acute and chronic care management, with an emphasis on patient-centered approaches, utilizing extensive clinical knowledge to optimize health outcomes. Requirements

  • Registered Nurse
  • Valid RN license with current registration
  • Experience in critical care settings preferred
  • Proven experience in Level I Trauma Centers or Level II Trauma Center environments highly desirable
  • Extensive background in ICU, emergency room, medical-surgical units, or post-acute care settings
  • Strong knowledge of physiology, medical terminology, HIPAA compliance, and diagnostic evaluation procedures
  • Expertise in discharge planning, utilization management, case management, and managed care processes
  • Experience with patient assessment techniques including vital signs monitoring and clinical documentation
  • Familiarity with hospital records systems and medical records management
