We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. The ideal candidate will coordinate patient care across various settings, ensuring seamless transitions from hospital to post-acute care and outpatient services. This role requires a strong foundation in acute and chronic care management, with an emphasis on patient-centered approaches, utilizing extensive clinical knowledge to optimize health outcomes.
