About this role

Registered Nurse (RN) Hospice Case Manager

Oakstone Hospice is seeking a compassionate and skilled RN Hospice Case Manager to join our interdisciplinary team. This role provides direct patient care, care coordination, and support to patients and families throughout the end-of-life journey.

Responsibilities

  • Manage a caseload of hospice patients in home and facility settings
  • Complete initial and ongoing nursing assessments
  • Develop and update individualized plans of care
  • Provide symptom and pain management focused on comfort and quality of life
  • Coordinate care with physicians, social workers, chaplains, CNAs, and other team members
  • Educate patients and families on medications, disease progression, and end-of-life expectations
  • Respond to changes in patient condition and provide timely interventions
  • Complete accurate, timely documentation per hospice and Medicare regulations
  • Participate in interdisciplinary team meetings

Qualifications

  • Active RN license in the state of practice
  • Minimum 1–2 years of nursing experience
  • Hospice experience preferred minimum of 1 year (training available for the right candidate)
  • Strong assessment, communication, and time-management skills
  • Ability to work independently in the field
  • Valid driver’s license and reliable transportation
