Position: On-Call Registered Nurse (RN) - Evening/Nights Department: Home Health & Hospice Supervisor: Hospice Clinical Manager Job Summary The On-Call Registered Nurse (RN) provides skilled, professional nursing care to patients, families, and caregivers as prescribed by the physician and in compliance with agency policies, state and federal regulations, and Medicare Conditions of Participation. The RN collaborates with the patient’s primary RN to reassess, update, and implement the Plan of Care, ensuring timely, comprehensive, and patient-centered interventions. The On-Call RN responds to after-hours calls, visits, and urgent patient needs while coordinating closely with the interdisciplinary team to support optimal patient outcomes and family satisfaction. Primary Responsibilities Clinical Care & Nursing Process * Conduct timely and thorough patient assessments addressing physical, emotional, and psychosocial needs. * Implement and evaluate nursing interventions, therapeutic procedures, and education for patients and caregivers. * Collaborate with physicians and interdisciplinary team members to revise the Plan of Care based on patient needs. * Provide education regarding disease progression, symptom management, and self-care techniques. * Document accurate, complete, and timely clinical notes consistent with regulatory requirements and agency standards. * Demonstrate competence with specialized procedures only when trained and credentialed. Interdisciplinary Collaboration * Communicate effectively with physicians, interdisciplinary team members, patients, and families to promote continuity of care. * Utilize community resources, volunteers, and agency staff to meet holistic patient needs. * Participate in interdisciplinary case conferences and contribute to the development of patient-centered goals. On-Call & Resource Utilization * Provide after-hours nursing coverage, responding promptly and professionally to urgent calls, visits, and hospice deaths. * Manage caseload and travel efficiently, demonstrating cost-effective care and compliance with reimbursement standards. * Maintain availability, accountability, and responsiveness during designated on-call shifts. Quality Assurance/Performance Improvement (QAPI) * Participate in agency performance improvement activities, clinical record review, and quality outcome initiatives. * Identify opportunities for process improvement and contribute to solutions that enhance patient care and satisfaction. Customer Service Excellence * Demonstrate empathy, compassion, and respect for patients and families. * Deliver care that upholds the agency’s values of Integrity, Respect, Teamwork, and Appreciation. * Maintain accountability for patient satisfaction throughout the episode of care. Secondary Responsibilities * Attend required staff, department, and interdisciplinary team meetings. * Participate in committees, trainings, and educational opportunities to support professional growth. * Perform other duties as assigned to meet agency and patient needs. Physical Requirements and Working Conditions * Communication: Strong verbal and written skills to interact with patients, caregivers, and staff. * Mobility: Ability to sit, stand, walk, bend, kneel, and drive frequently; lift or carry up to 50 lbs occasionally. * Environment: Ability to work in varied home and facility settings. * Transportation: Must maintain a valid Pennsylvania driver’s license, reliable automobile, and insurance coverage. Qualifications * Graduate of an accredited registered nursing program; Bachelor’s degree in Nursing preferred. * Minimum one year of professional nursing experience required; hospice, home health, medical/surgical, or critical care preferred. * Current RN license in the Commonwealth of Pennsylvania. * CPR certification required. * Strong ability to function independently while collaborating within an interdisciplinary team. * Satisfactory references, background check, and compliance with agency credentialing.
