Work Status Details: REGULAR FULL TIME | 80.00 Hours Every Two Weeks Shift: Days Pay Rate Type: Annual Salary Location: Yuma Medical Center Listed is the base hiring salary range offered for this position. Actual salaries may vary depending on factors, including but not limited to skills and experience. The salary range listed is just one component of the total rewards/compensation package offered to candidates. Min = $100,495.83 Mid = $130,644.58 Max = $160,793.32 Summary: The RN Complex Case Manager is an experienced healthcare professional responsible for coordinating care and discharge planning for patients with highly complex medical, psychosocial, behavioral health, legal, financial, and transition-of-care needs. This role serves as the lead coordinator for patients requiring intensive interdisciplinary collaboration, prolonged hospitalization, difficult discharge planning, or extensive post-acute resource coordination. The RN Complex Case Manager partners with patients, families, physicians, nursing staff, social workers, post-acute providers, payers, community agencies, and healthcare leaders to develop and implement individualized care plans that promote quality outcomes, safe transitions of care, patient-centered decision-making, and efficient utilization of available resources. Responsibilities: Complex Case Identification and Assessment: Receives and manages referrals from Case Managers for patients with complex clinical, psychosocial, behavioral health, financial, legal, guardianship, placement, or post-acute care needs that create barriers to discharge or continuity of care. Utilizes established screening criteria, risk stratification tools, and professional assessment to identify patients requiring advanced care coordination and complex discharge planning. Conducts comprehensive assessments of patient needs, support systems, social determinants of health, financial resources, discharge barriers, and post-discharge requirements. Evaluates patient conditions, prognosis, goals of care, and available resources to develop appropriate transition plans. Complex Care Coordination and Discharge Planning: Consults with physician and case manager on difficult cases and evaluates needs considering patient’s condition and prognosis. Facilitates patient and family understanding of clinical status through individual family conferences and offers alternative considerations within the patients’ and family means. Develops recommendations for disposition which are acceptable to patient, family, and physician and which are in accordance with all applicable state and federal regulations. Pro-actively negotiates appropriate discharge plans with post-acute providers who can accommodate the patient’s physical and medical requirements. Attends weekly care conference to improve communication between disciplines and to provide consulting input as needed. Aggressively seeks alternative funding resources to support discharge plan. Patient, Family, and Provider Collaboration: Consults with physicians, advanced practice providers, Case Managers, Social Workers, nursing staff, and other interdisciplinary team members on complex cases. Promotes, maintains, and seeks opportunities to further improve collegial relationship with community agencies and other professional disciplines by participating in community activities and serving as patient advocate. Demonstrates pro-active creativity in negotiating post-acute services with community providers and the finance department. Interdisciplinary Communication and Team Support: Works well under pressure of time and shifting priorities. Demonstrates skill in preparing written reports, progress notes and other material that is concise and well-constructed for ease of reading. Consults daily with program colleagues and assists where necessary to support program goals and achieve desired outcomes. Independently performs responsibilities and in organizing and prioritizing daily activities. Able to maintain flexible work hours to meet patient and family needs and to achieve program outcomes. Serves as a resource to physicians and health care team members. Demonstrates knowledge of current payor contracts, hospital-based policy / procedures, regulatory changes, etc. ensuring a high quality and safe standard of care. Maintains strict patient confidentiality. Professional Practice and Performance: Performs effectively in a fast-paced environment with competing priorities and complex situations. Participates in quality improvement initiatives designed to improve care transitions, reduce discharge delays, and enhance patient outcomes. Performs other duties as assigned.
