Responsibilities

Provide licensed practical nursing care to patients in a facility.

Monitor vital signs and report deviations to the supervising clinician.

Administer prescribed medications and document administration accurately.

Identify and escalate potential safety risks or clinical concerns promptly.

Assist with patient assessments and care planning under supervision.

Maintain meticulous patient records and ensure compliance with privacy standards.

Collaborate with the healthcare team to meet patient care targets and outcomes.

Educate patients and families on basic care, post-care instructions, and safety.

Participate in performance-driven initiatives and track personal metrics against targets.

Maintain clean and organized work environment; comply with infection control standards.

Assist with scheduling and prioritization to maximize efficiency and target attainment.

Support on-site training and mentorship for peers to foster growth.

Experience/Qualifications

Practical nursing license (LPN) active and in good standing.

Experience administering medications and documenting administration accurately.

Vital signs monitoring and prompt reporting of deviations.

Strong patient assessment and care planning under supervision.

Excellent clinical documentation and privacy/compliance with health records.

Performance-driven mindset with tracking of personal metrics and targets.

Infection control practices and a clean, organized work environment.

Ability to work on-site with strict attendance and schedule adherence.

Team collaboration and coordination with healthcare staff to meet patient outcomes.

Mentorship and training support for peers; willingness to share knowledge.
