Home Health Registered Nurse (RN) | Flexible Schedule | Full-Time, Part-Time, PRN | Cleveland Area

BrightStar Care is seeking an experienced Registered Nurse (RN) to serve as a key clinical team member delivering high-quality care throughout Cuyahoga, Lorain, and Medina counties. This role supports new client admissions, urgent staffing needs, skilled nursing visits, and ongoing case management.

Job Responsibilities Skilled Nursing Care Perform initial assessments, reassessments, and safety evaluations Deliver skilled nursing services including wound care, injections, infusions, and other treatments within RN scope of practice Monitor patient conditions and communicate changes appropriately Maintain accurate and timely clinical documentation Follow infection control and safety protocols Case ManagementCare Coordination Develop and maintain individualized Plans of Care Coordinate care with physicians, families, and interdisciplinary teams Support client admissions and transitions of care Ensure compliance with agency, state, federal, and Joint Commission standards Staff SupportSupervision Conduct supervisory visits and competency assessments Provide clinical guidance and education to caregivers and nursing staff Assist with orientation, skills validation, and ongoing training Support quality assurance and chart review activities Qualifications Required Current unrestricted Ohio Registered Nurse (RN) license Graduate of an accredited nursing program Minimum three (3) years of clinical nursing experience Minimum one (1) year of case management experience Current CPR certification Valid driver's license, reliable transportation, and auto insurance Ability to travel throughout assigned service areas Ability to pass background check, drug screening, and other pre-employment requirements Preferred Home health experience Healthcare management experience Pediatric nursing experience Experience supervising clinical staff
