An experienced Hospice Registered Nurses ready to hit the ground running in *Brown County, WI (Green Bay Area). *This contract role is perfect for someone who thrives on independence, embraces clinical challenges, and LOVES helping patients in the comfort of their own homes.
If you’ve got rock-solid _*HCHB (HomeCare HomeBase) skills and can manage a full caseload like a pro—this is YOUR opportunity.*_
*Schedule + Contract Details:*
- *Monday to Friday 8A-5PM*
- *On Call - NOT required*
- *Contract:* *13 Weeks to Start* | *High Chance of Extension*

Compensation
- Up to *$112,590 per year* -- *PLUS *_*mileage reimbursement & full benefits*_
- _*Expectations: *_5-6 patients/day.
- *Visits:* Routine, death, prn, change in condition.
- *Territory: *Approx up to 60 mile travel radius from branch.

Responsibilities

- Complete initial & comprehensive assessments to determine patient eligibility
- Create and implement individualized care plans with the patient’s provider
- Use sound clinical judgment and document accurately via *HCHB*
- Communicate regularly with the care team, scheduling, and leadership
- *Be the calm in the chaos—delivering compassionate, professional care while keeping the workflow moving efficiently*

- *2+ years working in Home Health Hospice setting visiting multiple patients per day!*
- *HCHB Experience (HomeCare HomeBase) REQUIRED*
- *Active RN license in Wisconsin or Nursing Compact License*
- *NOTE: *NLC or State License *MUST *be active at time of application, submission
