Peak to Peak Home Healthcare, LLC is a licensed and Medicare/Medicaid-accredited home health agency dedicated to providing high-quality skilled home health services in the Denver metro area and surrounding areas.

Peak to Peak Home Healthcare, LLC has established a reliable referral network with local hospital systems, rehabilitation and skilled nursing facilities, and physicians’ offices, and our census continues to outgrow our healthcare team. We are currently seeking highly skilled and compassionate Registered Nurse (RN) to join our growing healthcare team. The Registered Nurse will lead care by providing intermittent in-home nursing services, enabling full clinical autonomy and control over their scheduling and patient outcomes.

Requirements

  • Licensed as a Registered Nurse in Colorado with an active license in good standing with the Colorado Department of Regulatory Agencies.
  • Completion of a state-approved nursing education program.
  • BLS/CPR certified.
  • Comfortable using an EMR platform on a phone or tablet. Paid training is provided.
  • Valid driver’s license and a reliable vehicle.
  • Able to work effectively and independently in the patient’s home environment.
  • Excellent communication and interpersonal skills.
  • ONE YEAR OF EXPERIENCE AS A REGISTERED NURSE IN THE HOME HEALTH SETTING IS REQUIRED .
  • OASIS DOCUMENTATION EXPERIENCE IS REQUIRED .

Responsibilities

  • Perform thorough patient assessments, including vital signs, physical examinations, and detailed health histories, to develop individualized care plans.
  • Provide direct patient care and administer medications, injections, and IV infusions, and provide wound care with adherence to aseptic techniques and as indicated by the plan of care.
  • Assess patient needs and provide patient education.
  • Educate patients and their families on disease management, medication administration, lifestyle modifications, and post-discharge care to foster understanding and compliance.
  • Conduct specialized procedures, including medication management, wound care, IV/PICC care, catheter care, colostomy care, and laboratory specimen collection, while maintaining strict infection-control standards.
  • Maintain effective communication as part of an interdisciplinary team regarding patient care and patient progress, using various communication methods, including the phone, EMR platform, and communication application.
  • Maintain accurate and detailed patient records, including patient treatment and patient progress.
  • Participate in continuing education and professional development to stay current with nursing standards.
  • Maintain patient confidentiality and adhere to HIPAA guidelines.
  • Be a key member of an outstanding and supportive interdisciplinary team.

Experience and

Skills

  • Strong background and experience working in the home healthcare setting, especially with geriatric patients.
  • Proficiency in managing medical records and implementing infection-control measures.
  • Ability to work effectively in a fast-paced environment and prioritize tasks as required.
  • Strong communication and interpersonal skills to work effectively with other members of the multidisciplinary team, patients, and their families.
  • ONE YEAR OF EXPERIENCE AS A REGISTERED NURSE IN THE HOME HEALTH SETTING IS REQUIRED .
  • OASIS DOCUMENTATION EXPERIENCE IS REQUIRED .
