About this role

Provides professional nursing care as defined in the nurse practice act and under the direction of the Director of Clinical Services, Patient Care Coordinator, or Medical Director. Performs job in compliance with Agency policies and procedures and professional and community standards. Participates in coordination of patient care services (hospice and LTC), attends meetings as required, and follows reporting guidelines.

Demonstrates proficiency in use of agency chosen software, EMR, iPad documentation and other systems and technology as required. Conducts and documents completion of initial assessments accurately and within required time frames. Participates in development of the Plan of Care, as appropriate, for each assigned patient.

Works in diverse social and economic situations without imposing own values on patient and family. Explains hospice care and Keystone Hospice services and/or facility services and programs to prospective patients/families, physicians, and other appropriate individuals. Serves as a resource for all Hospice and/or facility staff to facilitate continuity of patient care and enhance the quality of patient care.

Travel between long-term care facilities and/or place of primary residence.

  • Hospice & Long-Term Care Registered Nurse EDUCATION & LICENSURE BACKGROUND Registered Nurse with current license to practice in Indiana. Bachelor’s degree in nursing preferred, but not required. Minimum of one (1) year clinical nursing experience; hospice experience preferred, but not required. Demonstrated knowledge of the regulatory requirements for hospice.
