About this role

As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love. Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.

Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals. Monitor patients' conditions and report changes. Educate patients and their families on disease management, medication, and treatment options.

Maintain accurate records of patient care and coordinate with other healthcare professionals. Oversee the work of Home Health Aides.
