Apex Health Solutions LLC is a licensed home care agency serving clients throughout Bergen County and surrounding areas in New Jersey. We are committed to delivering compassionate, high-quality, patient-centered care in the comfort of our clients' homes. We are currently seeking dedicated Registered Nurses (RNs) to join our growing team and provide skilled nursing services to private-pay clients. The ideal candidate is compassionate, reliable, organized, and passionate about helping individuals maintain their independence and quality of life at home.
Responsibilities
- Conduct comprehensive nursing assessments and ongoing patient evaluations.
- Develop, implement, and update individualized care plans.
- Monitor and document patient health status and progress.
- Administer medications and treatments as ordered by physicians.
- Educate patients and family members regarding disease management, medications, and home safety.
- Coordinate care with physicians, caregivers, and other healthcare professionals.
- Maintain accurate and timely clinical documentation.
- Identify changes in patient condition and communicate findings to the Director of Nursing.
- Ensure compliance with agency policies, state regulations, and professional nursing standards.
- Participate in care conferences and quality improvement activities as needed.
Qualifications
- Active New Jersey Registered Nurse (RN) License.
- Minimum 1 year of nursing experience preferred.
- Home care, hospice, geriatric, or community health experience preferred.
- Current CPR/BLS Certification.
- Strong assessment, communication, and documentation skills.
- Valid driver's license and reliable transportation.
- Ability to work independently and manage patient care effectively.
Pay: $52.00 per hour
