Licensed Practical Nurse

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. *Responsibilities*

• Deliver direct patient care by monitoring vital signs, administering medications, and performing clinical assessments with precision and compassion.
• Assist with complex procedures such as IV infusion, catheterization, wound care, and sterile techniques following established protocols.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs.
• Maintain accurate and detailed medical documentation using Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA and long-term care regulations.
• Conduct patient triage over the phone or in person, assessing urgency levels for emergency room or urgent care situations.
• Support discharge planning by educating patients and families on post-care instructions, medication management, and follow-up appointments.
• Ensure infection control standards are upheld through proper hand hygiene, sterilization procedures, and adherence to safety policies.

*Skills*

• Proven experience working with diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities, dementia or Alzheimer's disease, and those requiring hospice or memory care.
• Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques.
• Skilled in performing physical examinations, specimen collection/processing, venipuncture, phlebotomy, suturing, and airway management.
• Proficiency with EMR/EHR systems such as Epic, Cerner or Athenahealth; experience managing electronic health records efficiently.
• Ability to handle acute pain management cases in settings like ICU or Level I Trauma Centers; experience in emergency medicine or hospital medicine is highly valued.
• Excellent caregiver communication skills to effectively educate patients and coordinate with healthcare teams across settings including skilled nursing facilities and home & community care environments.
• Knowledge of infection control practices, HIPAA confidentiality policies, first aid procedures, and long-term care regulations.
