How You'll Make An Impact
Your Role
Ensures timely and efficient patient referrals and transition throughout the continuum of care from diagnosis to treatment to survivorship. Responsible for the management of resources and services through interdisciplinary collaboration to achieve optimal, contiguous patient outcomes for patients.

Associate degree in nursing

Two (2) years of clinical care, case management, or navigation experience

Florida registered nurse (RN) license
