Your Role as a RN

We are seeking a Registered Nurse (RN) Case Manager to provide skilled nursing care to our home health patients. This position is available as part-time , with flexible scheduling. Care locations would be primarily in Eagle Mountain & Saratoga Springs.

You’ll work closely with an interdisciplinary team to

  • Conduct admission and OASIS assessments
  • Perform head-to-toe and wound evaluations
  • Create and implement individualized plans of care
  • Provide follow-up skilled nursing visits
  • Educate patients and caregivers
  • Coordinate care with MDs, CNAs, PT/OT, and family members
  • Attend bi-weekly team meetings and contribute to care planning

Pay and Benefits

  • $50 to $105 per visit
  • Mileage reimbursement

Qualifications

  • Current Utah RN license
  • Reliable transportation
  • Excellent communication and interpersonal skills
