Responsibilities
Deliver high-quality, compassionate nursing care to residents in accordance with individualized care plans
Provide person-centered care to residents living with dementia and other memory-related conditions
Support residents with activities of daily living (ADLs) while promoting dignity, independence, and respect
Promote resident engagement, socialization, and meaningful activities to support mental and emotional well-being
Create and maintain a safe, secure, and supportive environment for residents
Follow the nursing process to assess, plan, implement, and evaluate individualized care
Monitor residents for changes in condition, behavior, mood, or overall well-being and respond appropriately
Accurately document resident history, vital signs, clinical observations, and changes in condition
Administer medications and treatments as ordered and in accordance with applicable policies and regulations
Communicate effectively with RNs, physicians, CNAs, CMTs, families, and interdisciplinary team members
Provide guidance and support to the nursing team to ensure consistent, high-quality resident care
Qualifications
Current LPN license in the state of Missouri (Required)
Previous experience in memory care, dementia care, long-term care, assisted living, rehab, or a similar healthcare setting preferred
Strong communication, clinical, organizational, and leadership skills
Compassionate and patient-centered approach to resident care
Ability to work effectively as part of a team
Commitment to providing safe, respectful, and individualized care
