Responsibilities may include: Completes an initial assessment of patient and family to determine home care needs; Provides a complete physical assessment and history of current and previous illness(es); Regularly re-evaluates patient nursing needs; Initiates the plan of care and makes necessary revisions as patient status and needs change; Uses health assessment data to determine nursing diagnosis; develops a care plan and include patient and family in planning process; Counsels the patient and family in meeting nursing and related needs; provides health care instructions to the patient as appropriate per assessment and plan of care; identifies discharge planning needs; acts as case manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload; Prepares clinic notes and updates primary physician when necessary; Participates in on-call duties as defined by the on-call policy; Provide direct patient care as defined in the California's Nurse Practice Act; implements plan of care; provides accurate and timely documentation consistent with plan of care; participates in coordination of home health services and appropriately reporting the identified needs for other disciplines (HHA/PT/OT/ST/MSW);

Qualifications include: Current License to practice professional nursing in the state; Maintain current CPR certification; Licensed driver with auto insurance in accordance with state requirements in good working order; complies with accepted professional standards and practices; prolonged or considerable walking or standing; ability to lift, position, or transfer patients; Able to lift supplies and equipment; demonstrates good verbal and written communication and organization skills; current professional liability insurance; physical exam and PPD test requirements on a yearly basis; provide proof of legal eligibility to work.

Prefer applicants with previous work history; may consider new grads.
