About this role
*Overview* We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. In this role, you will provide essential nursing care to patients, assisting them with daily activities and medical needs. The ideal candidate will have a strong commitment to patient care, excellent communication skills, and the ability to work collaboratively in a fast-paced environment.
*Duties*
- Provide direct patient care under the supervision of registered nurses and physicians.
- Conduct patient assessments and triage to determine the urgency of care required.
- Administer medications and as prescribed.
- Administer feeds and Meds via G-Tube
- Assist in the management of acute pain and support patients through various medical procedures
- Educate patients and their families on health management, including diet, medication, and equipment usage (DME).
- Collaborate with healthcare team members to develop and implement patient care plans.
- Maintain accurate patient records using electronic health record systems

- Proficient in triage techniques and patient assessment
- Ability to effectively communicate with patients, families, and healthcare professionals.
- Strong organizational skills with attention to detail in documentation.
