About this role

We are seeking a Licensed Practical Nurse (LPN) to join our Home Health Team in Charlotte County. This role provides skilled nursing care under the direct supervision of the Registered Nurse. Performs selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care. Is responsible and accountable for making decisions that are based on the individual's nursing experience and educational preparation.

This is a Pro Re Nata (PRN) position with variable time and hours can vary from week to week based on staffing needs, patient volume, call-offs, vacations, or organizational requirements.

What You'll Do

Patient Care: Provide high-quality nursing care, including administering medications, wound care, and vital sign monitoring. Clinical Assessments: Conduct comprehensive assessments to identify patient needs and develop individualized care plans. Medication Administration: Accurately administer medications and educate patients on their medication regimen. Treatment Implementation: Implement prescribed treatments, such as IV therapy and catheter care. Patient

Education

Provide patient and family education on disease management, prevention, and self-care. Collaboration: Work collaboratively with physicians, therapists, and other healthcare providers to ensure optimal patient outcomes. Documentation: Maintain accurate and timely documentation of patient care, assessments, and interventions. Infection Control: Adhere to strict infection control guidelines to protect both patients and healthcare workers. Emotional Support: Offer emotional support and compassion to patients and their families during challenging times.

What We're Looking For

Licensure

Current and valid LPN license in the state of Florida.

Experience

Minimum of 6 months of clinical experience as an LPN in a hospital or skilled nursing facility.

Skills

Excellent communication, interpersonal, and critical thinking skills. Professionalism: Strong work ethic, reliability, and a commitment to providing quality care. Transportation: Reliable transportation to visit patients in their homes.
