CNA / Home Health Aide Responsibilities

Depending on the client's care plan and your qualifications, responsibilities may include:

  • Providing compassionate personal care and assistance with activities of daily living
  • Assisting clients with bathing, grooming, dressing, toileting, and mobility
  • Supporting meal preparation and feeding as appropriate
  • Assisting with transfers and ambulation
  • Providing companionship and emotional support
  • Observing and reporting changes in the client's condition
  • Maintaining a safe, clean, and comfortable environment
  • Following the client's individualized plan of care
  • Maintaining accurate and timely documentation
  • Communicating appropriately with the client, family members, and healthcare team

LVN Responsibilities

LVNs will provide nursing services within their scope of practice and in accordance with applicable Texas requirements, the client's plan of care, and Integra Healthcare Services policies.

Responsibilities may include

  • Providing skilled nursing care in the client's home
  • Assessing and monitoring client status
  • Administering medications and treatments as ordered
  • Monitoring vital signs and reporting significant changes
  • Providing wound and other nursing care as applicable
  • Educating clients and caregivers regarding the plan of care
  • Communicating changes in condition to the appropriate healthcare provider
  • Completing accurate clinical documentation
  • Collaborating with physicians, RNs, caregivers, and other members of the healthcare team
  • Maintaining compliance with agency policies, applicable regulations, and professional standards

Qualifications

CNA / hha:

  • Current CNA certification and/or HHA qualification as applicable to the position
  • Previous home care experience is preferred but not always required
  • Dependable, compassionate, and professional
  • Strong communication and interpersonal skills
  • Ability to work independently in a client's home
  • Ability to meet all applicable employment and agency requirements

*LVN:*

  • Current, unrestricted *Texas LVN license*
  • Previous home health or clinical experience preferred
  • Current CPR/BLS certification as required
  • Strong clinical, communication, and documentation skills
  • Ability to work independently while collaborating effectively with the healthcare team
  • Ability to meet all applicable employment and agency requirements
