REGISTERED NURSE--HOME HEALTH/HOSPICE 7-on/7-off On-Call

This position would join a pool of on call nurses to share the load. You will provide care on a 7-on/7-off schedule.

• Medical, dental, vision, life insurance, supplemental insurance, 401K match, and PTO/Holiday Pay for full time staff.

*on call nurse essential functions*

• Takes call from 5:00pm to 8:30am M-Th, and Friday 5:00 pm to Monday 8:30am.

• Field/triage patient/family calls and concerns

• Conducts routine visits and/or admit visits with home health or hospice patients

• Assures thorough team communication regarding patient status at shift end

• Provides community education to potential patients and their families

*on call RN qualifications*

• Cheerful customer service oriented.

• Current Utah RN license (required).

• Experience in the home health and hospice settings.

• Excellent availability and response time to calls.

• Demonstrated ability to triage and apply critical thinking skills.

• Knowledge of and ability to navigate electronic medical records. HomeCare HomeBase experience a plus.

• Reliable transportation is required.
