About this role

Compass Healthcare Consulting & Placement is conducting a search for a Registered Nurse Care Manager for patients in 1 location, an Assisted Living Facility located in Brooklyn, NY. Qualified candidates will be familiar with Home Health RN Care Management, UAS Assessment and Home Care Services. Position is Monday - Friday, Days. Base salary Plus additional compensation for each UAS Assessment Completed!

About the position

The RN Care Manager (Registered Nurse) performs UAS Assessments and case management to patients in the assisted living facility. The RN advocates for quality patient care, collaborates with Physicians and other providers to maintain a safe and effective environment for patients. The Registered Nurse works closely with patients and interdisciplinary team to ensure quality care is provided within the facility. Completed an Average of 20 UAS Assessments per Month, all in 1 location.

Qualifications

Must be a NYS Registered Nurse Clinical RN experience required. ie. Home Care, Assisted Living Facility, UAS Assessment experience Ability to commute to assisted living facility in Brooklyn, NY Willingness to collaborate with Interdisciplinary Team as needed Excellent Organizational Skills Required.

This is a Full Time Direct Hire Position offering Benefits and Competitive Base Salary $106,000 plus $150 per completed UAS Assessment.
