POSITION SUMMARY RN Case Manager

POSITION DUTIES Support and facilitate a smooth transition from the acute care setting or SNF to alternative levels of care of home

MINIMUM REQUIRED QUALIFICATIONS GA license

PREFERRED QUALIFICATIONS Kaiser experience

LENGTH OF ASSIGNMENT 13 weeks

Shift / hours per week m-f; 8:00-5:00 eow
