Under the general supervision of the Director of Nursing, the Rehabilitative Services Associate (RSA) has a 7.5-hour shift and collaborates with team members that include the Shift Nurse and (Q)MHP. Work schedule may include occasional weekends, holidays and irregular hours when necessary or warranted.

• Provide clinical support in collaboration with persons served as a resource demonstrating and teaching measurable skills necessary to complete activities of daily living (ADL).

• Responsible to collect data regarding Residents’ progress surrounding the ADL training provided to Residents by the Interdisciplinary Team (IDT.)

• Arrive 15 minutes before the start of each shift to give shift report. Punch in and out of the time clock at the beginning and end of each shift, break, and meal.

• Consult the Communication Log within first hour of shift.

• Complete rounds with prior/next shift to check for completion of assignments to impart information and events regarding residents. Coordinate and collaborate with all RSAs and nurses on the same shift.

• Utilize the Communication Log to report resident behaviors and successes, daily.

• Complete accurate hourly, 1:1, 15-minute and 30-minute rounds.

• Answer call lights immediately or ensure a peer RSA/Nurse is attending to the resident, communicate with peers on the floor or call from first floor for assistance. Call Light should not be engaged without attention for more than one minute. Floor Nurse and all floor RSAs should be able to communicate as to whom is attending to Call Light.

• Follow Universal Precautions in giving care.

• Take vital signs and weights as assigned by nurse and reviewed in the 24-hour shift report.

• Encourage residents to go for medications during medication pass.

• Pass trays during meals, collect trays after meal and chart food intake.

• Monitor residents during meals for choking, conflicts and behavioral issues. Report any incidents or pertinent observations to floor nurse immediately after meal, such as a person refusing to eat.

• Facilitate improvement of skill set for Activities of Daily Living to assigned person served. This may be in 1:1 assigned by (Q)MHP, group training, 1:1 requested by resident.

• Assist person served with bathing/hygiene/other ADLs as needed and follow any bathing/toileting/hygiene program of person served, as needed.

• Assist with bed making, or makes beds, changing linen as needed. Dust rooms: nightstands, dressers, walls, etc.

• Report to maintenance any broken or malfunctioning lamps, furniture, doors, sink, toilet, tiles, etc.

• Assist the person served to ensure they maintain their room in a clean and neat condition.

• Use encouragement and motivational interviewing along with skills training to provide support. For additional support, seek assistance from Nurse or (Q)MHP.

• Note specific shift duties, however all shifts may assist in the following:

a. (7AM-3PM) Assist person served with closets: clean, neat, 18” below ceiling, floor clear, belongings in proper place. Each person served is allowed 50% of the space.

b. (3PM-11PM) Assist person served with dresser drawers and night table: clean, organized, free of trash and contraband, tops wiped. Floor clear of trash and belongings. Assist person served to remove all soiled clothing and linens to the laundry chute.

c. (11PM-7AM) Monthly cleaning of hallway per schedule.

• Accompany person served to medical appointments outside of the facility, including possible visits to Highland Park Hospital Emergency Department. If person served consents, go into the examination room to answer any questions the doctor may have. Collect visit summary from Doctor and report to the nurse upon return.

• Inventory belongings upon admission. Update inventory when person served brings new items to the facility or receives new items by mail. Check in with Resident quarterly to ensure the form is complete and updated. Report to (Q)MHP when belongings become an issue and occupy more than 50% of room or when items are prohibitive to maintaining a clean and tidy room.

• Conduct and/or coordinate new employee and new resident orientations.

• All nursing staff is responsible to purchase, wear, clean and maintain uniforms.

• Cell phone use is restricted to breaks and lunch.
