Performance Home Health (PHH) is seeking experienced and motivated Part-Time Registered Nurses (RN) to join our home health team. The Home Health RN is responsible for providing skilled nursing care to patients in their homes while meeting PHH clinical, documentation, and productivity expectations. This role requires strong clinical skills, accountability, and the ability to work independently while maintaining consistent communication with clinical leadership and the interdisciplinary team. Part-time home health RN. Salary starting at $60,000/year.
Schedule
Friday-Sunday or Saturday-Monday. 18 productivity points weekly. Commercial insurance only - No OASIS documentation. Wound care, wound vac, picc care, port care and more. Direct patient care, care coordination, and provider communication. Perform comprehensive patient assessments, including Start of Care (SOC), recertifications, and discharges. Develop, implement, and update individualized plans of care in coordination with physicians and the interdisciplinary team. Provide skilled nursing services such as wound care, wound vac management, port care, injections, catheter care, IV therapy, picc line care and patient/caregiver education.
