Regional Advanced Practice Provider (O-6 Billet) Supervisory

Through the direction of the Chief APP, the incumbent carries out APP program goals, objectives, priorities, and guidelines. Serves as subject matter expert and consultant for national standards and policies, and for urgent, short-term, and long-term planning to meet the agency needs. Critically reviews healthcare data, reports, and prepares reports of findings as required by the Chief APP.

Serves as subject matter expert, author, editor for agency reports, policies, and programs as required despite dynamic format and deadline requirements. Serves as member of APP senior clinical leadership and advisor to the IHSC Chief APP for all matters related to APP practice. Supports the Chief APP by serving in the acting role as needed.

Assists in the credentialing of APPs, peer review process, medical assistant program, and the MAST program as directed by the Chief APP. Participates in development, implementation, and revision, of orientation, training, and electronic health record tools for APPs. Represent the agency for APP related care and authors written or verbal communication in contact with other government agencies including local, state, and federal entities.

Possesses high degree of professionalism and tact and the ability to coordinate complex and/or controversial communications that address high-level agency concerns including responses to legal challenges. Assists Chief APP by providing expertise for policy, standards, programs, and critical agency missions that involve APPs. Maintains working knowledge of both nurse practitioner and physician assistant credentialing and collaborative practice requirements.

Regional supervision includes developing, planning, and critically evaluating the clinical practice and skills as it relates to APPs in the incumbent's region. Supervision: Provides APP oversight in their respective region. Provides oversight for PHS, federal civil servants, and contract employed APPs.

Reviewing official for PHS and GS APPs in their respective region. Subject matter expert for evaluation of APP clinical practice, job performance, or misconduct concerns. Participates in recruitment, hiring, retention, and recognition of APPs.

At the direction of the Chief APP, works with Contracting Officer Representative (COR) and vendor leadership as required to address government contracting concerns that affects all contract employed APPs in IHSC. Judgement failure can result in ineffective or inappropriate use of resources and delays in meeting public health objectives of the program or the agency. This may impair development of national guidance, affect clinical care, and compromise patient safety.

Failure can also result in loss of credibility and embarrassment for the agency. The purpose of work contacts includes collaboration with all agency components to advance agency strategic goals, address clinical care challenges, and provide high-quality, efficient, and effective patient-centered healthcare for ICE non-citizens: Performs other duties as assigned to support the agency mission. Position may require travel up to 30% of the time in support of the mission to include site visits, site clinical support, mission critical staffing, and travel to IHSC headquarters.

PHYSICAL DEMANDS: IHSC operates 24/7/365. Response is required during an emergency activation. Average manual dexterity for computer operation. Tele and video conference call meetings for extended period of time. May require rotational on-call duties and work during evenings, weekends, and holidays.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES: 1. Educational

Qualifying degree for the officer's category from an accredited institution in a State, the District of Columbia, or a U. S. territory. The degree issuing institution and/or program must be accredited by an entity recognized by the U. S. Department of Education Physician Assistant minimum: Master's degree. Nurse Practitioner minimum: Master of Nursing. 2. Unrestricted, current, and valid license from any U. S. state, the District of Columbia, Puerto Rico, or other U. S. territory. It is the responsibility of the incumbent to fulfill and maintain the obligation(s) for their licensing or certifying body and to maintain continuing medical education.3. National

Physician Assistant - Must have and maintain certification as a Physician Assistant from the National Commission on Certification of Physician Assistants (NCCPA). Nurse Practitioner - Must have and maintain board certification as Family Nurse Practitioner by the American Nurse Credentialing Center (AANC) or American Academy of Nurse Practitioners (AANP). 4. Must have and maintain CPR/BLS certification.

5. Must have and maintain clinical practice proficiency and credentials for patient care. 6. Must possess at minimum six years of full-time clinical experience as a physician assistant or nurse practitioner or approval of the IHSC Medical Director. 7. Must possess at minimum two years of team lead or supervisory experience.

Demonstrated ability to perform O-5 Supervisory duties. 8. Must be able to attain and maintain DHS security clearance. 9. Flexibility and ability to adapt to sudden changes in schedules and work requirements.

10. Knowledge of a wide range of complex patient care concepts, principles, and practices to perform assessments of considerable diversity to include mental health, medical surgical, ambulatory and emergency care. 11. Possesses a high degree of independence and initiative with minimal oversite. Resolves complex, sensitive, national issues and work product must conform with multiple accrediting and guiding standards.

12. Strong writing skills to include document formulation, editing, review along with intermediate or advanced knowledge of Microsoft Office programs. Preferred, but not required: Experience supervising clinical staff within the IHSC health system. Experience or training in government contracts.

Experience in clinical leadership of correctional healthcare at regional or national level. Certification as Correctional Healthcare Professional (CCHP). Knowledge and/or experience with IHSC policies, National Commission on Correctional Health Care (NCCHC) and ICE Performance-Based National Detention Standards (PBNDS) requirements.

Continued Supervisory Controls Work is reviewed for attainment of objectives. Guidelines may be applicable to some but not all parts of assignment and may contain some inconsistencies and be partially unconfirmed. Selection and adaptation from available possibilities involving a moderate amount of modification and innovation is required.

The incumbent uses judgment in interpreting and adapting guidelines such as PHS policies, regulations, precedents, and work direction for application to specific cases. The incumbent analyzes results and recommends changes in guidelines and program policies. Innovations and modifications to accepted procedures must be approved and clearly documented.

The incumbent keeps the supervisor informed of the status on projects and consults with him/her on any significant problems encountered.

This U. S. Public Health Service position is located within the Department of Homeland Security (DHS), Immigration and Customs Enforcement (ICE), Enforcement and Removal Operations (ERO), ICE Health Service Corps (IHSC), Division of Clinical Services, Medical Services Unit. The incumbent serves as the Regional Advanced Practice Provider (APP). The incumbent reports directly to the Chief Advanced Practice Provider.

This is a supervisory position. The IHSC APP national program facilitates the provision of healthcare services provided by nurse practitioners and physician assistants across the agency to non-citizens in ICE Custody. The incumbent is within this program and serves as a supervisor in the medical services chain of command within the assigned geographic region.

The incumbent works collaboratively with the regional clinical and operational leadership to provide clinical guidance, consultation, and mentoring for Advanced Practice Providers (APP). Responsibilities include participation in all areas of the IHSC national program development and maintenance, with emphasis on clinical onboarding, training, program development, communications, policy development and revision, advocacy, recruitment, hiring, and retention. The incumbent may have periodic clinical duties in support of the IHSC mission and as assigned by the Chief APP.

The incumbent serves as the Acting Chief APP when required. IHSC provides direct care daily to approximately 15,300 detainees housed at 20 designated facilities throughout the nation. The health care provided includes medical, dental, mental health care, and public health services.

IHSC provides medical case management and oversight for an additional 22,600 detainees housed at approximately 112 non-IHSC staffed detention facilities daily across the country. In addition, IHSC oversees the financial authorization and payment for off-site specialty and emergency care services for detainees in ICE custody. The ICE detainee population is approximately 34,000 detainees on a daily basis, with an average length of stay of approximately 30 days, surpassing 400,000 detainees annually.

IHSC provides medical support during ICE enforcement operations in the air, on the ground and at sea. SUPERVISORY CONTROLS: The incumbent is supervised by the Chief APP. The incumbent receives direction to plan and implement national program priorities from the Chief APP.

Direction is received through direct personal contact, email exchanges, and established policy and precedents. While being supervised by the Chief APP, the incumbent functions within their region independently to ensure planning, development, implementation, and evaluation of projects or provision of complete services; may serve as advisor and subject matter expert for several projects in several professional fields. Situations to which existing guidelines cannot be applied or which require significant deviations from existing guidelines are referred to higher authority. Assignments are usually of a long-term, recurring or broadly defined nature. Incumbent carries out work given by Chief APP.
