Your Role as a Home Health RN: Perform comprehensive patient assessments in the home setting, Develop and manage individualized care plans, Provide skilled nursing care and patient/caregiver education, Administer treatments and monitor patient condition, Coordinate care with physicians and interdisciplinary teams, Evaluate patient progress and adjust care as needed, Complete accurate, timely documentation using streamlined tools
