Requirements
Current unrestricted RN license in Michigan.; BLS Certification required; Minimum 2 years of clinical nursing experience, with a background in case management preferred.; Certification in Case Management (CCM) preferred.; Proficiency in using telehealth platforms and electronic health records (EHR) systems.; Excellent communication skills, both verbal and written.; Strong critical thinking and problem-solving abilities.; Ability to work independently and as part of a multidisciplinary team.; Compassionate and empathetic approach to patient care.; Flexibility to adapt to changing healthcare env
