Active, Unencumbered State License (NP/APRN) in the State in which you are applying.; Active, Individual DEA with address listed in the State in which you are applying (please note that if you intend on prescribing in multiple states, a separate DEA is required in each state in which you are planning on prescribing).; DEAX designation as formerly required to prescribe Buprenorphine *or* proof of 8hrs of qualified training as assigned by SAMHSA will be required.; 1 yr.
