Per visit pay in home health: how do you compare it to an hourly offer?

The first year boards on allnurses have been full of this one: an agency quotes pay per visit, another quotes hourly, and a hospital quotes a salary, and the three refuse to line up on paper.
If you work per visit, what does a realistic week add up to once driving and documentation time are counted in? And what would you ask before signing that you didn't know to ask?

The comparison only works in one direction: convert everything to a week. A per visit rate times a realistic visit count, minus unpaid windshield time and charting, gives a weekly gross you can set beside an hourly schedule. Never annualize a per visit number; the visit count is the variable, and it's the number the agency controls.
Three asks that decide the math: how a visit counts when the patient cancels at the door, whether drive time and documentation are paid separately, and what a low census week looked like on that team this year.
