At Longleaf Hospice and Palliative Care, we integrate and address the diverse, seen and unseen needs of patients and their families.

The RN Case Manager is responsible for providing nursing care to patients, including identifying and coordinating patient/family care with other members of the Interdisciplinary Team and health care providers.

Responsibilities

  • Completes an initial assessment of patient and family to determine hospice care needs; provides a complete physical assessment and history of current and previous illness
  • Uses health assessment data to determine nursing diagnosis
  • Triages calls and assists patient/primary caregiver with problem resolution
  • Teaches and counsels patients and families in meeting nursing and related needs
  • Makes visits for symptom management and support as needed
  • Modifies patient treatment plans as indicated by patients' responses and conditions
  • Maintains current, accurate, detailed reports in accordance with policy
  • Monitors, records and reports symptoms and changes in patients' condition to the IDG
  • Provides health care instructions to the patient as appropriate per assessment and plan
  • Obtains and receives physician orders as required
  • Pronouncement of patients

Requirements

  • Current license to practice as a registered professional nurse in Georgia
  • Current and continued CPR certification
  • At least one or two years of recent home care experience; hospice experience preferred
  • Good verbal and written communication and problem solving skills
  • Responsible, valid driver’s license; reliable, insured, automobile for making home visits required
