Salary range: $33.69 - $47.55 per hour
How you'll make an impact in this role
- Participate in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care and update changes as necessary.
- Attend Interdisciplinary Team (IDT) care plan meetings and Participant/Family meetings as applicable. May perform assessments in a variety of settings including participant's place of residence, PACE Clinic and/or Adult Day Center.
- Develop and provide education to participants, caregivers, and associates as necessary including end of life discussions. May require travel to participant's place of residence, long term care and assisted living facilities.
- Assess participants in their homes for home care needed to support their independence living in the community.
What minimum qualifications you'll need
Licensure / Certification / Registration
Required Credential(s)
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
- Registered Nurse obtained prior to hire date or job transfer date.
Education
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
