About this role
The Admissions Nurse performs initial Start of Care (SOC) visits including determination of appropriateness for home care, overall assessment, medication reconciliation, development of plan of care, and provision of direct care and treatments. Responsible for OASIS evaluations per CMS guidelines, coordination of services, supervision of home health aides, and maintaining documentation quality standards. Requires current NY State RN license, degree from accredited nursing school, valid driver's license and insured vehicle, OASIS Certification or willingness to obtain within 1 year, and minimum 1 year Home Health Care experience.
