LVN ( Licensed Vocational Nurse)

• Provides direct patient care and implements a plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Performs direct nursing care including skin, bowel, bladder programs, nutrition ,hydration, medications and ordered treatments, transfers, NG tube and gastrostomy tube feedings.
• Knowledge of assessment and documentation, spinal cord injury, wound care and staging, g-tube placement and care, tracheostomy care and ventilator experience.
• Positioning and wheelchair safety. Uses equipment and supplies effectively and efficiently.
• Participates in patient and family education.
• Reporting the identified needs for other disciplines (OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
• Utilizes all available resources in planning care and consults as needed with RN’s, medical staff, outside specialists or consultants, and other members of the interdisciplinary team.
• Continuously reviews, evaluates, and documents the effectiveness of nursing care, and accomplishment of client goals for discharge.

Performs other duties as assigned by the registered nurse and/or Clinical Supervisor.

Compensation is based on experience.
