Responsibilities
- Provide skilled nursing visits in patients’ homes
- Complete assessments, care plans, and patient education
- Perform wound care, medication management, and coordination of care
- Communicate with physicians and the interdisciplinary team
- Document accurately and on time in a Medicare home health environment
Requirements
- Current Hawaii RN license
- 1+ year clinical experience (home health preferred)
- Strong assessment and documentation skills
- Valid driver’s license + reliable transportation
- Ability to work independently in the field
