The role and schedule

- Traditional Home Health and Hospice is a locally owned Agency which provides skilled care to homebound home health patients and a holistic model of care to patients with a limited life expectancy.
- Day to day
- assess patients and develop care plans with physicians, supervisors, patients, and families Assess patients and develop personalized care plans with physicians, nursing supervisors, patients, and families.

Your Role as an RN Case Manager

What You Need to Succeed
- Active RN License in Pennsylvania

- Acute Care and/or Home Health Experience (Preferred)
