The Mental Health Association of the Southern Tier (MHAST) is seeking a compassionate and skilled Registered Nurse (RN) Care Coordinator for Second Shift to join our Crisis Stabilization Center (CSC).
Our Crisis Stabilization Center operates 24 hours a day, 7 days a week, providing immediate support to individuals experiencing mental health and substance use crises. As a dually certified OMH/OASAS facility, we provide a safe, therapeutic environment where individuals can receive care, stabilize, and connect with community resources to support their recovery.
The RN Care Coordinator plays an essential role in providing medical care, conducting assessments, monitoring client health, and coordinating services. Working alongside a multidisciplinary team, this position helps ensure that every individual receives quality, person-centered, and trauma-informed care.
Key Responsibilities
Conduct nursing assessments, triage, and medical screenings to determine appropriate levels of care.
Monitor clients' medical status, including vital signs, symptoms, and overall health.
Provide basic nursing care, crisis interventions, psychoeducation, and de-escalation support.
Collaborate with psychiatric providers and the multidisciplinary team to address mental health and substance use concerns.
Assist in developing and implementing individualized service plans, safety plans, and discharge plans.
Coordinate referrals and connections to mental health treatment, substance use services, primary care, and community resources.
Address barriers to recovery by connecting clients with resources for housing, transportation, food, financial assistance, and other essential needs.

Someone on clinicals may say something that has you wondering if you are cut out for this. Do not take it to heart. A couple of bad days are not the profession.
