Summary
The Hospice RN Case Manager provides compassionate, high-quality care to patients and families facing terminal illness. This role coordinates all aspects of the hospice patient's care, ensuring comfort, dignity, and emotional support in accordance with the hospice philosophy and regulatory standards. The RN Case Manager serves as the primary point of contact for the patient, family, and interdisciplinary team. Key Responsibilities - Patient Care Coordination: Conduct comprehensive initial and ongoing assessments of patients' physical, emotional, and spiritual needs.
