About this role
Actively participates in multi-disciplinary rounds to review changes in patient status, progression and level of care, and discharge plans for assigned patients. Communicates with and educates patients and families regarding emotional, social, and financial impacts of illness. Assesses readmitted patients, organizes patient and family care conferences, documents discharge planning evaluation, and develops discharge plans with appropriate contingency plans. Requires Associates of Nursing, Registered Nurse license, and 2+ years medical/hospital nursing experience.
