As a Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes, following physician orders and Medicare guidelines.

Key responsibilities include performing SOC, ROC, Recertification, and Discharge visits, completing comprehensive assessments, providing skilled nursing services including medication management, wound care, patient education, and chronic disease management, completing OASIS and clinical documentation, collaborating with physicians and interdisciplinary team members, and educating patients and caregivers.
