Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.
**Title:** PRN Hospice Afterhours Visit RN
**
Schedule
** PRN, varies. Mostly
- 12 hour shifts overnight on weekdays. Some 12 hour weekend day shifts available.
**Location:** Field-based, varies throughout the territory based on patient need. Mileage reimbursement is provided at 72.5 cents per mile
**Pay:** $42.34 to $50.81 per hour
**essential duties and responsibilities**
- The Afterhours Registered Nurse will primarily conduct afterhours home visits and provide care to patients, as delegated by the Phone Triage Nurse.
- Provides nursing care to meet the patient’s needs in accordance with the patient’s hospice plan of care.
- Cross-train on phone triage to assist Triage RN team as necessary.
- Correctly identifies, communicates, and collaborates on patient problems/priorities/issues based upon assessment data. Establishes measurable patient goals and communicates an on-going plan of care appropriately to the interdisciplinary team (IDT). Recognizes changes in the clinical status of the patient and reports findings to the appropriate team member.
- Collaborates with patient/family when developing the plan of care, adjusting communication style as necessary.

- Must have a minimum of one year RN experience – hospice or home health experience highly preferred
- Must successfully pass required background checks and drug screening
- Requires travel to multiple work locations
