The Registered Nurse (RN) Navigator provides individualized support to patients, families, and caregivers to help them successfully navigate the healthcare system and overcome barriers to care. This role enhances the patient experience and clinical outcomes by ensuring a seamless, coordinated care journey across the continuum.

The RN Navigator collaborates closely with physicians and interdisciplinary care teams to triage, coordinate, and manage patient care, serving as a consistent point of contact and trusted resource for patients and families. The RN Navigator supports shared decision-making and patient engagement by building strong, trust-based relationships; addressing communication and health literacy needs; and helping patients integrate their values and preferences into treatment decisions. This role also contributes to strategic initiatives within the clinical program or service line, including the development, implementation, and evaluation of navigation activities to support program sustainability.

Licensure

RN-Registered Nurse of California, BLS-Basic Life Support Healthcare Provider. Knowledge of medical terminology required. Ability to ensure follow through with responsibilities and communicate with a varied level of health care staff.

Knowledge of third-party reimbursement helpful. Verbal and written communication skills. Microsoft Office software experience required, including Excel and Word.
