Skilled Nursing Care
- Perform comprehensive nursing assessments during home visits
- Provide skilled nursing care according to physician orders and individualized care plans
- Monitor patient conditions and identify changes requiring intervention or escalation
- Deliver high-quality, compassionate care while promoting patient comfort and independence
Infusion Therapy
- Administer and monitor infusion therapies safely and accurately
- Assess IV sites and monitor for complications or adverse reactions
- Educate patients and caregivers on infusion-related care and safety
Wound Care
- Perform wound assessments and dressing changes
- Implement wound care treatment plans according to physician orders
- Monitor wound healing progress and report concerns appropriately
- Educate patients and caregivers on wound management and prevention techniques
Patient EducationCare Coordination
- Educate patients and families on medications, treatment plans, disease management, and recovery goals
- Collaborate with physicians, case managers, caregivers, and interdisciplinary healthcare teams
- Support care transitions from hospital or facility settings to home
DocumentationCompliance
- Complete accurate and timely clinical documentation
- Maintain compliance with agency policies, state regulations, and professional standards
- Follow infection control, safety, and HIPAA guidelines at all times
Qualifications
- Active and unrestricted North Carolina Registered Nurse (RN) License
- Minimum of 2 years of RN experience required
- Infusion therapy experience required
- Wound care experience required
- Emergency Room (ER) experience preferred
- Strong clinical assessment and critical thinking skills
- Ability to work independently in a home care environment
- Excellent communication and patient education skills
- Professional, compassionate, and dependable demeanor
