Job Description
We are seeking a Registered Nurse (RN) for a home health assignment with Franciscan Healthcare in West Point, Nebraska. This position follows a Monday–Friday schedule from 8:00 AM to 4:30 PM , with a 30-minute lunch built into the shift. The assignment supports both Home Health and Hospice patients, and candidates with experience in both areas are preferred.
The current caseload includes home health and hospice patients, with new referrals evaluated based on staffing and individual patient needs. Weekend coverage is required 1–2 weekends per month , which may consist of on-call coverage or scheduled visits depending on patient needs, including wound care or hospice patients requiring increased support.
The RN will also participate in on-call coverage Monday through Friday from 4:30 PM to 8:00 AM , approximately 2–3 days per week, as well as weekend call from Saturday at 8:00 AM through Monday at 8:00 AM . The position requires a clean driving record , as the facility has a fleet of vehicles available for nurses to use. Candidates should be comfortable providing patient care in a home health and hospice setting and working independently while coordinating with the broader care team.
