About this role

Hospice of the Piedmont is a mission-driven organization dedicated to providing exceptional end-of-life care and support to patients and families throughout our community.

We are seeking a part-time Registered Nurse to join our Night, Evening, and Weekend Services (NEWT) Team. This position offers a consistent schedule, working two 12-hour daytime shifts each weekend.

As a NEWT RN, you will play a vital role in ensuring patients and families receive timely support outside of regular business hours. Responsibilities include responding to triage calls, assessing patient needs, providing clinical interventions and in-home visits as appropriate, and coordinating care with providers, vendors, and the interdisciplinary team. The successful candidate will bring strong assessment skills, sound clinical judgment, and the ability to remain calm and compassionate while managing urgent patient and family needs.

Triage and Patient Care

  • Respond to HOPs Triage service calls and maintains a log of calls and visits.
  • Make home visits to assess patient needs, provide nursing care and emotional support.
  • Perform nursing evaluations for new admissions and determine eligibility.
  • Provide specialized nursing services according to agency guidelines.

Communication and Reporting

  • Observe and report changes in patient needs to physicians and Hospice staff.
  • Confer with supervisors for guidance and inform them of any challenges.
  • Document appropriately and timely.
  • Maintain patient confidentiality.
  • Attend staff and team meetings.
  • Support the performance evaluation process and provide coaching and feedback

Qualifications

  • Valid license as an RN in the Commonwealth of Virginia

Typical Physical Demands/Working Conditions

  • Drive long distances
  • Must be able to work in home environments that have difficult working conditions including, but not limited to cramped space, pets, dirt, allergens, smoke, no chairs
  • It may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care
  • Communicate by phone, writing, typing, speaking
  • While performing the duties of the job, the employee is regularly required to use hands to handle or feel objects, reach, kneel, walk, stand, balance and talk to hear.
  • Able to push, pull, or lift 50 pounds, push up to 200 pounds in a wheelchair
  • Must be prepared to complete EMR at bedside in varied environments
