VITAS Healthcare is hiring a Hospice Admissions Registered Nurse to conduct comprehensive clinical assessments to determine hospice eligibility, educate patients and families about hospice services and Medicare benefits, collaborate with physicians, case managers, and interdisciplinary teams, provide emotional support and clinical guidance during the decision-making process, and initiate and complete the admission process including documentation and coordination. Located at Good Samaritan Hospital and in field Santa Clara County.

Requirements
- RN License
- Minimum 2 years of nursing experience
- Hospice, palliative, ER, ICU, home health or case management experience preferred
- Strong clinical assessment and critical thinking skills
