Heart Failure RN Coordinator

This position provides clinical expertise and coordination of care for patients and families under direct or indirect supervision of a physician to ensure optimal patient care. This position may be required to access and administer medications within their scope of practice and according to State Law. Responsibilities include conducting new patient interviews, monitoring patient lab values and progress, providing patient/family education, coordinating patient discharge, acting as liaison between patient and medical providers, participating in clinical pathway development, contacting insurance companies, providing follow-up after discharge, and collaborating with multi-disciplinary teams.
