About the Role
Outfield Healthcare Partners is seeking a dedicated Wound Care Nurse (RN or LPN) to join our skilled nursing team in Independence, Missouri . This role is responsible for leading skin integrity and wound management programs to promote healing, prevent breakdown, and ensure regulatory compliance.

Position Summary
The Wound Care Nurse is responsible for assessing, treating, documenting, and monitoring residents with wounds, pressure injuries, and skin integrity concerns. This position collaborates closely with nursing leadership, physicians, and interdisciplinary teams to ensure best practices and positive outcomes.
Key Responsibilities
Perform comprehensive skin and wound assessments
Obtain and carry out wound care treatment orders
Monitor wound healing progress and recommend treatment changes
Document assessments, treatments, and outcomes accurately
Track pressure ulcers and skin alterations, including weekly measurements and reports
Ensure completion and accuracy of Braden risk assessments
Implement and monitor preventive interventions for at-risk residents
Educate nursing staff on wound care protocols and best practices
Coordinate wound care supplies with central supply
Assist DON and Medical Director with case reviews related to pressure injuries
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
Stay current on state, federal, and industry wound care standards
Qualifications
Graduate of an accredited RN or LPN program
Active nursing license in the state of Missouri (required)
Minimum 1 year wound care experience in a long-term care setting preferred
2+ years of skilled nursing facility experience preferred
Current BLS/CPR certification
Strong assessment, documentation, and communication skills
Knowledge of skin integrity programs, prevention strategies, and regulatory requirements
